15 Years of Advancing Care for People with Serious Illness and Their Caregivers

For 15 years, the Coalition to Transform Advanced Care (C-TAC) has united leaders across health care, government, advocacy, philanthropy, and the patient community to transform care for people living with serious illness. Through bipartisan policy leadership, cross-sector collaboration, and an unwavering commitment to person-centered care, C-TAC has helped shape the national conversation and advance meaningful improvements for patients and families.

Join us as we celebrate this milestone and look ahead to the next chapter of innovation and impact. The C-TAC National Leadership Event & 15 Year Celebration will convene national leaders to reflect on the progress made, recognize the champions who have helped drive change, and explore the opportunities that lie ahead.

Throughout the day, attendees will:

  • Highlight the policy milestones, innovations, and partnerships that have transformed care.
  • Elevate the voices and lived experiences of patients, families, and caregivers.
  • Engage in forward-looking discussions on the future of person-centered, integrated care.
  • Strengthen partnerships and inspire collective action to expand access to high-quality, compassionate care for all.
  • Celebrate 15 years of advancing care for people living with serious illness.

Together, we will honor the past, celebrate the present, and chart the course for the next generation of serious illness care.
