Strategic Plan + Goals
The time is now.
The care system is failing to provide the high quality care that patients and their family caregivers need. And in the next 10 years, the number of people needing care will only increase — dramatically.
The U.S. population is aging rapidly with over 10,000 people becoming Medicare-eligible every day. And by 2030, the number of Medicare enrollees is expected to double to approximately 80 million individuals.
That’s why C-TAC set a big “Moonshot” goal for 2030: to make sure that all 12 million people living with serious illness can have a high quality of life.
Yes, it’s a big challenge, but we’re making progress.
More people are starting to understand the need for change, especially with stories of public figures like President Jimmy Carter facing serious illness or caregivers like movie star Bradley Cooper. And big ideas are on the table for national and state policy change, from support for community infrastructure to testing new payment models for serious illness.
Advocacy
Our Strategic Plan consists of two parts
Part 1: Fixing the Broken System.
We are working with Medicare and Medicaid to use C-TAC core principles to help both patients and unpaid caregivers during serious illness. Some of the important steps in this plan are:
- Providing more money for services in the community, like transportation, meals, and social activities.
- Making care planning and decision-making easier and fair for everyone.
Please join the growing Coalition to support better services for patients and caregivers by calling for policy changes such as: Getting rid of co-pays for planning future care and supporting states that are adding Medicaid benefits for social services.
We appreciate our members’ ideas and help with our policy goals. Today, with your help, more people are getting support for serious illness as a result of our advocacy.
Part 2: Mobilizing a Growing, Diverse Coalition.
With over 200 members we are finding the best ways to care for people and are teaming up with policy makers, health plans, clinicians and others to share these models. Progress will look like:
- Health plans and others using these care models
- Supporting our partners as they train the workers who provide this care
- Making sure patients and their unpaid caregivers have what they need to make good decisions and plans
Ready to help? Join us in creating reports about care models and other useful research (like the Compendium, ACT Index, and more) with partners such as the American Heart Association, USAging, the Center to Advance Palliative Care, different health plans, health systems, and community leaders and faith groups.
