Redefining Care
We all want the best care, especially when facing a serious illness.
That’s why our goal is to find and help spread care models that are working today – and make sure they are available for everyone.
What does the best care look like? The care models we are looking for meet one or more of the Core Principles we’ve outlined. And you can access resources where we’ve published our findings.
And we are also supporting the changemakers who are delivering care through workgroups and projects in the following areas:
- Enhancing the capacity of trusted community leaders in the Aging Network, Faith Community, and those addressing social isolation
- Supporting unpaid caregivers
- Mobilizing health plans and other payors to share best practices
- Working with patient advocacy groups to improve access to care for heart failure, cancer, dementia and other serious illnesses
