New Report on Care Models across the Care Continuum
Creating a continuum of care from diagnosis is our top goal. What’s good care look at different stages of an illness, delivered in different settings, under different payment models, by different teams? There’s clearly a lot of complexity to cut through! In the spirit of providing you with some orientation through this complexity, we’re excited to release a Compendium of Care Models that will be a great resource for those looking to:
- Embed serious illness programs within broader, risk-based population-health payment models where many populations are included
- Reach populations with serious illness at home
- Address health equity through faith-based and other community partnerships
- Meet the needs or pediatric populations facing serious illness
The Compendium is organized around the key challenges that decision-makers are facing when determining whether to invest in or scale programs focused on serious illness care.
