C-TAC Innovations is a 501(c)3 non-profit organization affiliate of C-TAC established in 2016 to guide organizations and communities to effectively implement and sustain equitable, evidence-based models of care for persons with serious illness.
C-TAC Innovations recently announced that Respecting Choices is joining WiserCare. The Respecting Choices programs have been within C-TAC Innovations for the past six years. The time is right to expand reach and access to advance care planning and decision support through this new relationship! Stephanie Anderson, DNP, RN will remain the executive director of C-TAC Innovations, and C-TAC Innovations will continue its current work, and expand on this in the future, working closely with communities and organizations across the country.
C-TAC Innovations work includes, but is not limited to, broad-based stakeholder engagement and other projects that will improve care for people living with serious illness and their family caregivers. Two examples of this work are the Expanding Community-Based Palliative Care Services project in Hawai‘i, and the recently launched Expanding Access to Palliative Care Services and Supports for Adults, Children, and their Caregivers in New York State. Both projects focus on improving health equity for the medically underserved, low income, and minority populations.
