C-TAC Member Meet-Up: Pivoting from Fee-for-Service to Value-Based Models

Join leaders from Empassion and Good Shepherd Community Care to discuss opportunities for Primary Care at Home and Palliative Providers to pivot from fee-for-service models and thrive in value-based arrangements, including CMS’ ACO REACH High Needs Model.

Empassion is an MSO that enables community-based providers to deliver a complete care solution to provide more good days for patients with advanced illness and their families.

Good Shepherd Community Care is dedicated to the delivery of the highest quality of care, support and education for those experiencing or anticipating serious illness and loss.

 

Speakers:

Terri Maxwell, PhD, RN

As Empassion’s Chief Clinical Officer, Terri Maxwell PhD, RN is responsible for developing and evaluating clinical care delivery models, professional development, and quality. Terri practiced as an advanced practice nurse in oncology for over 20 years before focusing her career on improving care for persons with serious or advanced illness. Prior to joining Empassion, she co-founded and led a national home-based population health palliative care company, Turn-Key Health that was acquired by CareCentrix. In addition, she established the Center for Palliative Care at Thomas Jefferson University in Philadelphia, where she led a health system-wide palliative care quality initiative and developed a medical school and residency palliative care curriculum. She also led clinical initiatives, research and education for Hospice Pharmacia, a national hospice medication management company that later became Enclara Pharmacia. Terri received a bachelor’s degree in nursing from the University of Rhode Island and a master’s and PhD from the University of Pennsylvania where she received the 2020 award for Nursing Innovation. She is a Soros Faculty Scholar for the Project on Death in America, and a widely published author, frequent presenter at national meetings, and served on the board to develop the Advanced Practice Certification exam for Hospice and Palliative Care Nurses.

 

Jenna Ackerman

Jenna Ackerman is the Senior Director of Growth and Strategy for Empassion Health, where she leads Empassion’s strategic business development efforts. Prior to Empassion, Jenna was a Director of Corporate Strategy for VillageMD, a national risk-bearing primary care provider. She also spent several years in management consulting, primarily working with health systems on the development and operationalization of their value-based care strategies. Jenna received her MBA in Healthcare Management from the Wharton School at the University of Pennsylvania.

 

Chelsea Shenker, MSW, LICSW, ACHP-SW, APHSW-C

Chelsea Shenker is the Director of Palliative Care for Good Shepherd Community Care, where she has spearheaded the organization’s exploration of value-based arrangements for palliative care patients. She currently oversees all of Good Shepherd’s palliative care programs, which include pediatric, adult inpatient and adult community-based palliative care programs. As a healthcare leader and administrator, clinical social worker, and educator, she has practiced in diverse settings in palliative care, oncology, and hospice. She has a particular interest in care for veterans and those with trauma histories in the setting of advanced illness. Chelsea holds a Master of Social Work from Boston College and has credentials in Advanced Palliative and Hospice Social Work (APHSW-C) and Advanced Certified Hospice and Palliative Social Work (ACHP-SW).
