Humana Foundation Health Equity Innovation Fund Awards C-TAC Multi-Year Grant

Humana Foundation logoThe Humana Foundation, the philanthropic arm of Humana, Inc. based in Louisville, KY, recently awarded C-TAC a three-year grant to advance its regional health equity mission to support seniors by creating emotional connectedness and healthy lives. C-TAC will build on its foundational work in Louisville to identify and deploy trusted community leaders to connect seniors in underserved communities to community health workers.

C-TAC has launched several pilots that leverage faith leaders to connect seniors with chronic conditions to community health workers, based on the assumption that faith leaders are trusted community leaders. The purpose of these pilots is to improve clinical outcomes for seniors by expanding the range of trusted leaders beyond faith leaders only. To validate this work, CTAC plans to develop tools to assess who in the community qualifies as a trusted leader and to measure the effectiveness of different types of leaders in connecting seniors with community health workers. These tools will also evaluate retention in the Community Health Worker Model (CHW) and whether trusted leaders play a role in improvement the overall outcomes of nutrition or mental health for seniors.
