Brief

Briefs are short briefing documents about an single issue, like a policy brief.

How can we build, expand and more sustainably fund CBOs so that more people with serious illness, especially those in marginalized communities, can receive the comprehensive, community-based care they need?

How would a more equitable healthcare approach better address the full range of patient needs and be more responsive to the differences in their environment, finances, culture, and overall well-being?

How can we leverage the flexibility, speed and innovation of state models of care to influence federal policy decisions?

What measures can be taken to eliminate the cost-sharing burden that prevents this patient population from taking advantage of the benefits advanced care planning provides?

This document provides recommended quality measures for a palliative care or serious illness initiative, along with high-level implementation considerations, such as what can be collected from claims versus provider reporting.

Shirley Roberson

In July 2020, C-TAC made a commitment to advancing health equity, recognizing that it was embedded in our vision, but never fully acted on.

We defined equity for our work as the transformation of Care for all Americans living with serious illness, with a particular focus on achieving the highest level of anti-racist care for those who are the sickest, most vulnerable and disadvantaged.

Census Bureau shows a gradual decline in telehealth utilization among adults and children between April and October 2021.

C-TAC has historically focused on improving access to advance care planning (ACP) as the foundation of person-centered care and it is a key priority for our 2022 policy efforts.

CAPABLE is a person-directed, home-based program that addresses both function and healthcare expenses.

C-TAC’s successful legislative and regulatory work of 2021.

C-TAC has developed the Core Principles for Care Models for Those with Serious Illness to provide clear guideposts that can be used to evaluate payment and care models.
