A website-based toolkit seeks to help health care organizations participating in value-based payment models — including accountable care organizations and Medicare Advantage plans — adopt evidence-based interventions to improve outcomes and lower overall costs of care for individuals with complex needs. This is a collaborative between seven leading health care foundations to bring the best evidence-based practices and promising approaches to improve care for people with complex health and social needs.
