Addressing the multiple determinants of health and closing the gap on health equity will require cross-sector collaboration within our health ecosystem. This report covers recommendations and best practices for screening and intervening on the social determinants of health as well as a strategy for assessing and addressing population health disparities, especially from racism. It addresses five focus areas for health care organizations to address social determinants of health and health equity: Planning, Identification, Tracking & Measurement, Follow-Up, and Incentives & Investments.
