Member Meetup Recap – Home Centered Care Institute (HCCI)

As health care becomes more important, the needs of people with serious illness have become more diverse. To reach effective advanced illness care, Home Centered Care Institute (HCCI) introduced and discussed the model of Home-Based Primary Care (HBPC) to improve the quality of life for patients and their caregivers.  

On August 10th, 2022, Home Centered Care Institute (HCCI) officially introduced the model of Home-Based Primary Care (HBPC) at the C-TAC member meet-up. The session was presented by Paul Chiang, MD, Senior Medical and Practice Advisor, HCCI, and Medical Director, Northwestern Medicine HomeCare Physicians; Melissa Singleton, MEd, Chief Learning Officer; HCCI, and Dana Crosby, Senior Director, Engagement and Practice Development, HCCI.  The team explained the HBPC’s general idea, payment reform, and the need for work development in the meeting. 

“I think of advanced illness care in a nutshell, the modern-day house call,” said Paul Chiang, the senior Medical and Practice advisor in HCCI, when talking about the value and the idea of Home-Based Primary Care: “We really need to involve, not only home hospice and home-based palliative care but home-based primary care as well.” 

The main purpose of Home-Base Primary Care is to improve the quality of life for medically complex and / or seriously ill patients and their caregivers. HBPC also tries to prevent avoidable hospitalizations and nursing home placements by promoting the model of care. It aims to include better outcomes, improved patient experience, lower cost, and greater job satisfaction for providers and staff. 

Two main clinical care models are transitional care and longitudinal care. Home-Based Primary Care can partner with PCP providers to take care of younger people with chronic illnesses or older people with high use of hospitals.  

When talking about how Home Centered Care Institute helps the model, Dana Crosby, the Senior Director of HCCI, explains three ways: “First and foremost, through education, we offer a multi-model curriculum and topics that span the full breadth of home-based care. We also have a team of expert consultants to improve efficiencies and enhance reimbursement to bridge gaps in care. HCCI has a platform that we call ERIN for home-centered care. And it’s a unique and interactive business intelligence tool that integrates multiple data sources and that can aid in some decision-making.” 

Home-Based Primary Care can vary significantly among clinical providers, health care managers, and administrators, as well as the public. It is about relationships with patients, their family members, and their caregivers. Being in the home provides the model with unique chances to identify risks.  

 “I think there’s an increasing awareness, as well as that Home-Based Primary Care really aligns with value-based care,” said Paul Chiang: “That is changing the landscape of our country here.” 

“C-TAC members are on the front lines when it comes to caring for patients with serious illness, and since many of those patients require longitudinal primary care at home, HCCI is a natural partner,” said Melissa Singleton, HCCI’s Chief Learning Officer. “We have seen growing support and acceptance of the fact that home-based primary care is an essential component of advanced illness care.”
