C-TAC will host a series of convenings of community faith leaders that will build upon the foundation of C-TAC’s work in Louisville, Detroit, and Ward 7 in DC. This initiative harnesses existing momentum and draws on the collective expertise of faith leaders from around the country. Together, they will create a toolkit and roadmap to improve the health and well-being of people with serious illness and their family caregivers in under-served and under-represented communities. The toolkit will include methods, resources, and case studies on successful models of community engagement for other communities to use and benefit from.
