The new leadership at CMS and CMMI, has published their 10-year vision on value-based care. These leaders support equity as a standalone principle for all CMS models. Now is the time to include community-based services across these existing care models to address the medical, social, spiritual and support needs of those living with serious illness. Our roundtable of experts address C-TAC’s “Core Principles” for those models and the case for addressing equity by supporting community-based models and the pathway to lasting success.
