About Hope Health Care

At Hope Health Care, our main focus is to deliver care that patients and families talk about, care that matters. In a world where uncertainty abounds, one thing patients, families, community partners and stakeholders can count on, is Hope Health Care.  Since 1983, the Medicare Hospice Benefit has been accepted by greater numbers of patients, families and Healthcare Providers as the “Gold Standard” for end-of-life care in the United States.  At Hope Health Care, our desire is to help recipients of the care not to equivocate hospice with death, but with life.  It’s time to re-think hospice!

 

Our Team

The Hope Health Care team seeks to make every patient story matter.  Our team consists of Physicians, Registered Nurses, Licensed Practical Nurses, Social Workers, Chaplains, Volunteers and Support Staff to bring the health care you need, to where you live.

 

Our Difference

What makes Hope a bit different?  One practical difference is our approach to spiritual care.

With one of every four deaths in the United States being the loss of another Veteran or their spouse, Hope Health Care focuses additional resources in meeting Veteran needs.  We not only participate in the We Honor Veterans Program through the National Hospice & Palliative Care Organization, but we take it a step further by having a full-time chaplain who is a Veteran himself caring for Veterans receiving our care.

It’s this investment in the Hope care delivery model that allows for this spiritual care advisor/professional to understand firsthand, the importance of what a veteran may have faced in a combat zone, being a combat survivor himself.  With four tours of duty in combat zones, this gentleman provides a resource of understanding that allows Veterans to hear a direct voice that may help them deal with the often unverbalized “soul wounds” that they often deal with.

 

Texas Represents

Hope Health Care has been a voice for serious illness in the DFW metroplex with its Founder being elected to the Texas New Mexico Hospice Organization in 2017 to serve on the TXNMHO Executive Board and in 2019, being elected President of the Board and serving on the Board through 2023.  Holding National Memberships over time with The National Hospice & Palliative Care Organization, has allowed this Hope Health Care leader to represent the State of Texas through the Council of States.

 

Why C-TAC?

Hope Health Care is a family of caring companies that include home health, community based palliative care, hospice care and primary care joining together to improve patient care experience through the health care continuum.  Hope Health Care; in particular, joined C-TAC to continue to have the opportunity to learn from best known practices and voices in the National Conversation on setting the tone for improvements in the patient care experience, and the opportunity to share the practices that our collective organizations have found to make a difference in the lives of those that we serve.
