
C-TAC has a long-standing partnership with USAging, the national organization representing Area Agencies on Aging (AAAs) and advocating for the Title VI Native American Aging Programs. Current collaboration includes an exciting project to support expanded advance care planning (ACP) opportunities for older adults as part of The John A. Hartford Foundation funded program called Advancing Organizational Equity and Excellence through Sustainable Cross-Sector Partnerships. C-TAC is in the midst of identifying community-based models currently in use at AAAs and elsewhere that provide or access advance care planning services for older adults. This environmental scan of ACP programs will lead to the development of a tool kit and educational and training resources for community-based organizations. C-TAC’s and USAging’s shared goal is to support the expansion of ACP at the local level to help ensure that older adults’ care preferences are followed during their care for a serious illness.
