C-TAC, along with an amazing group of messaging experts, participated in the development and release of a serious illness messaging toolkit designed to empower all of us who work to improve the lives of patients and caregivers. With this toolkit, your organization will be empowered to better capture public interest, bypass misconceptions, and increase demand for your services with better messaging. In this toolkit, we’ll show you easy ways to improve your messaging – or you can use the included messages that have been tested to work.

This toolkit was created during a three-year grant generously funded by The John A. Hartford Foundation and Cambia Health Foundation. We leaned heavily on McCabe Message Partners, and the information design you’re seeing now was created by Constructive.co.
The group conducted a scoping review of the literature, convened a stakeholder design process to develop the principles, tested the principles in focus groups before and during the COVID pandemic, and collaborated with a couple of national research projects. In addition, almost everything in this toolkit has been road-tested by our organizational partners below, and we’ve pored over their messages, designs, and analytics.
The John A. Hartford Foundation is dedicated to improving the care of older adults. Since 1982, The John A. Hartford Foundation has awarded more than $625 million in grants, investing in aging experts and practice innovations to enhance the health and well-being of older people. The Age-Friendly Health Systems Initiative is a movement to help spread the reliable delivery of age-friendly care, based on the 4Ms framework (what Matters, Medication, Mentation, Mobility), to all healthcare settings.
The Cambia Health Foundation is dedicated to transforming the healthcare journey by connecting changemakers to the resources and knowledge they need to maximize their impact. Founded in 2007, the Foundation has funded over $80 million in grants to advance whole-person care models at every stage of life to build a just and inclusive healthcare system for all.
