C-TAC Hosts a Webinar on Advance Care Planning with Leading Researcher and Advocate, Dr. Rebecca Sudore

C-TAC recently convened members and national experts for a timely discussion on the evolving landscape of advance care planning (ACP), along with updates on state, federal, and congressional priorities affecting people living with serious illness and their caregivers.

Opening the webinar, Tom Koutsoumpas, C-TAC Co-Founder and Co-Chair, welcomed participants and underscored C-TAC’s continued commitment to advancing person-centered care and ensuring that the needs, goals, and preferences of individuals and families remain at the center of serious illness care.

Advancing Meaningful Advance Care Planning

Dr. Rebecca Sudore, Clinical Research Scientist at the University of California, San Francisco (UCSF), and Founder and Director of PREPARE, shared insights into making advance care planning more accessible, effective, and person-centered.

Her presentation highlighted the importance of moving beyond advance directives alone and creating opportunities for people to understand their health care choices, identify what matters most to them, and communicate those preferences with their families and care teams. Effective advance care planning is an ongoing process that can help ensure individuals are better prepared to make medical decisions as their health needs change.

Scott Brown, President and CEO of MyDirectives, continued the conversation with a look at how technology can help strengthen advance care planning. Brown discussed opportunities to make individuals’ care preferences more accessible and actionable across care settings, helping ensure critical information is available when and where it is needed.

Together, the presentations reinforced the importance of combining meaningful conversations, accessible tools, and interoperable technology to help ensure that care reflects each person’s goals and values.

C-TAC Policy and Advocacy Updates

The webinar also provided members with updates on C-TAC’s work to advance serious illness care across the public and private sectors.

Torrie Fields, MPH, C-TAC Strategic Advisor, provided a state and private payer perspective, highlighting opportunities to strengthen access to person-centered serious illness care through evolving payment and delivery models.

Marian Grant, DNP, ACNP-BC, ACHPN, FPCN, RN, C-TAC Senior Regulatory Advisor, shared the latest federal regulatory developments and their implications for palliative care, advance care planning, and the broader serious illness community.

Brian Lindberg, C-TAC Senior Public Policy Advisor, rounded out the policy discussion with an update on C-TAC’s federal policy priorities and congressional engagement.

These updates reflect C-TAC’s continued work across federal and state policy, payment and delivery reform, and stakeholder engagement to build a health care system that better supports people living with serious illness and their caregivers.
