Improvements in advanced illness care can and should be driven by collaborations on the
community level. With the advent of C-TAC’s State-Level Advanced Care Transformation Index, we can measure our progress on a variety of metrics and adjust accordingly to ensure that we are supporting high-quality care for people with advanced illness. This session will explore opportunities for cooperation and innovation between health systems and community advocates.
