Dr. Kristofer Smith Joins C-TAC as Senior Medical Advisor

Dr Kristofer SmithWe are pleased to announce that Kristofer Smith MD, MPP has joined the C-TAC team as Senior Medical Advisor. Dr. Smith is a leader in developing care models that improve the lives of those with serious illness. He joins the leadership team during a dynamic time when there is tremendous opportunity to scale these models nationwide.

A partner to C-TAC for more than a decade, Dr. Smith brings a wealth of experience to our leadership. He was a core member of the team that developed the C-TAC Serious Illness Model, key elements of which were adopted by the CMS Innovation Center and other payors and providers. As the SVP of Population Health at Northwell, he built one of the nation’s leading home-based primary care programs and CMMI demonstration site for Independence at Home. He was the Chief Medical Officer at Landmark Health and Prospero Health, both leading companies providing primary, urgent, and palliative care to frail seniors. As a board certified Interest and Palliative Care physician, he worked for more than a decade providing home based primary and palliative care to patients in New York City and Long Island.

Dr. Smith is also active in research and advocacy. He published papers on serious illness care models for the frail elderly and regularly gives national talks on health policy, the frail elderly, as well as the intersection of payment reform and clinical redesign. He currently serves on the board of the American Academy of Home Care Medicine and the Elea Institute.
