The Arizona Coalition to Transform Serious Illness Care has developed a new model for delivering home and community-based palliative care. The new model aligns with our Core Principles for Care Models and exhibits transformative progress toward a health system that better meets the complex needs of all people living with serious illness.
In collaboration with C-TAC, Discern Health, and Arizona End of Life Care Partnership, the Arizona Coalition to Transform Serious Illness Care is led by Arizona Hospital and Healthcare Association (AzHHA) and convenes a group of more than 40 stakeholders across Arizona dedicated to improving the lives of people with serious illness. The development of this new model exemplifies the dynamic work occurring at the state-level across our field.
