CMMI and Medicaid Programs are experimenting with care models for people with serious illness more often than ever before. Between the launch of ACO REACH, GUIDE, Enhancing Oncology, and Cardiac Bundled Payment by CMMI and expanded access to care for Health-Related Social Needs and Palliative Care services by Medicaid programs, there is tremendous opportunity for health plans and other payors to incorporate C-TAC’s care model principles for people with serious illness into their approach to delivering care.
