Sandra P. Gomez, MD FAAHPM

Dr. Gomez is a graduate of the University of Washington School of Medicine and has over 20 years of clinical experience. She is double boarded in Family Medicine and Palliative Medicine, operated one of the largest private palliative medicine physician practices in Texas and is passionate about hospice and palliative care. During her time at Memorial Hermann, the largest not-for-profit health care system in Southeast Texas, she developed and implemented 8 inpatient community palliative care programs. For this work, her teams received the American Hospital Association’s Circle of Life Award and the International Hospital Federation’s Quality and Patient Safety Award. She is a former adjunct faculty at MD Anderson Cancer Center and currently sits on the board of directors for Bristol Hospice and the AAHPM. Her prior hospice experience includes direct patient care, nursing and provider education and inpatient unit development and implementation. Recently during her time at Optum, she helped to develop and integrate several community programs for over 120 thousand Medicare Advantage Beneficiaries in the Houston area. The programs focused on social determinants of health, care management, community health workers, and outpatient palliative care. She is currently the Medical Director of Provider Training and Care Model Innovation for Sage Health, a concierge senior care practice focused on Medicare Advantage patients in Alabama, Mississippi, Arkansas, and Maryland. She currently lives on a 10-acre farm with her husband of 20 years and 5 children ages 8 to 19.
