Coordinated Care Alliance (CCA) is an innovative statewide network of 24 trusted community-based organizations (CBO) in Illinois.

C-TAC member organization Hucu.ai is empowering the Coordinated Care Alliance of Illinois (CCA) to provide seamless care transitions for hundreds of seriously ill patients in Illinois annually.

Food & Friends provides medically tailored meals for those with serious illness. Though shown to improve health outcomes and cost savings, this services is not often financed by Medicare and Medicaid.

A recent study published in the Journal of Social Science and Medicine points to the effectiveness of community-based palliative care (CBPC) in increasing care quality and reducing costs.

How can we build, expand and more sustainably fund CBOs so that more people with serious illness, especially those in marginalized communities, can receive the comprehensive, community-based care they need?

C-TAC is partnering with the Petrie-Flom Center at Harvard Law School to explore the opportunities for states to bring a broad range of palliative care services to seriously ill persons, particularly through changes to their Medicaid programs.

The Evidence Bank is a collection of research studies on common services provided by CBOs that demonstrate outcomes attractive to a health care partner. This collection is updated on a monthly basis.
