DESCRIPTION:Join leaders from Empassion and Good Shepherd Community Care to discuss opportunities for Primary Care at Home and Palliative Providers to pivot from fee-for-service models and thrive in value-based arrangements\, including CMS’ ACO REACH High Needs Model.  \nEmpassion is an MSO that enables community-based providers to deliver a complete care solution to provide more good days for patients with advanced illness and their families. \nGood Shepherd Community Care is dedicated to the delivery of the highest quality of care\, support and education for those experiencing or anticipating serious illness and loss. \n  \nSpeakers: \n \nTerri Maxwell\, PhD\, RN \nAs Empassion’s Chief Clinical Officer\, Terri Maxwell PhD\, RN is responsible for developing and evaluating clinical care delivery models\, professional development\, and quality. Terri practiced as an advanced practice nurse in oncology for over 20 years before focusing her career on improving care for persons with serious or advanced illness. Prior to joining Empassion\, she co-founded and led a national home-based population health palliative care company\, Turn-Key Health that was acquired by CareCentrix. In addition\, she established the Center for Palliative Care at Thomas Jefferson University in Philadelphia\, where she led a health system-wide palliative care quality initiative and developed a medical school and residency palliative care curriculum. She also led clinical initiatives\, research and education for Hospice Pharmacia\, a national hospice medication management company that later became Enclara Pharmacia. Terri received a bachelor’s degree in nursing from the University of Rhode Island and a master’s and PhD from the University of Pennsylvania where she received the 2020 award for Nursing Innovation. She is a Soros Faculty Scholar for the Project on Death in America\, and a widely published author\, frequent presenter at national meetings\, and served on the board to develop the Advanced Practice Certification exam for Hospice and Palliative Care Nurses. \n  \n \nJenna Ackerman \nJenna Ackerman is the Senior Director of Growth and Strategy for Empassion Health\, where she leads Empassion’s strategic business development efforts. Prior to Empassion\, Jenna was a Director of Corporate Strategy for VillageMD\, a national risk-bearing primary care provider. She also spent several years in management consulting\, primarily working with health systems on the development and operationalization of their value-based care strategies. Jenna received her MBA in Healthcare Management from the Wharton School at the University of Pennsylvania. \n  \nChelsea Shenker\, MSW\, LICSW\, ACHP-SW\, APHSW-C \nChelsea Shenker is the Director of Palliative Care for Good Shepherd Community Care\, where she has spearheaded the organization’s exploration of value-based arrangements for palliative care patients. She currently oversees all of Good Shepherd’s palliative care programs\, which include pediatric\, adult inpatient and adult community-based palliative care programs. As a healthcare leader and administrator\, clinical social worker\, and educator\, she has practiced in diverse settings in palliative care\, oncology\, and hospice. She has a particular interest in care for veterans and those with trauma histories in the setting of advanced illness. What could the transformation of advanced care look like for patients and caregivers navigating serious mental illnesses? How can improving the experience of serious mental illness improve the experience of caregiving and experience of care when diagnosed with a serious illness? \n  \nJoin C-TAC Member COMPASS Pathways and C-TAC’s Strategic Advisor\, Torrie Fields for February’s Member Meet Up. \n  \nSpeakers: \n \nDr. Steve Levine  \nDr. Steve Levine is a board-certified psychiatrist and SVP of Patient Access at COMPASS Pathways\, as well as co-founder and chairman of the board of Heading Health.  Steve was formerly the founder and Chief Medical Officer of Actify Neurotherapies\, pioneering the clinical use of IV ketamine for treatment resistant depression\, treating over 6\,000 patients and providing more than 60\,000 infusions in 10 locations in 8 US states since 2011.  Steve completed his psychiatry training at New York-Presbyterian Hospital Weill Cornell Medical Center\, followed by subspecialty training in psychosomatic medicine/psycho-oncology at Memorial Sloan-Kettering Cancer Center and New York Presbyterian. \n \nTorrie Fields\, MPH \nTorrie Fields serves as C-TAC’s Strategic Advisor. Torrie is also the Founder and serves as the Chief Executive Officer of Votive Health\, a company expanding access to care for people with serious illnesses by facilitating value-based arrangements between payers and preferred providers. Prior to Votive Health\, Fields oversaw serious illness strategy and value-based care at Blue Shield of California and Cambia Health Solutions. Fields’ experience has encompassed work as an economist and population health researcher in a variety of settings\, including health plans\, health delivery systems\, and local and federal health departments. She has deep expertise in policy development and implementation\, translating evidence-based research into legislative concepts and regulatory change. SUMMARY:C-TAC Member Meet-Up - Focus on Family and Friend Caregivers for Older Adults Living with Dementia
DESCRIPTION:This C-TAC member meet-up will highlight Benjamin Rose’s focus on caregivers\, with special attention to its efforts to develop\, disseminate\, and deliver evidence-based caregiving support programs across the US. \n  \nSpeakers:  \n \nOrion Bell\, CEO \nOrion Bell is President and CEO of Benjamin Rose Institute on Aging\, a nationally recognized Cleveland-based nonprofit whose mission is to advance support for older adults and caregivers.   He has more than 30 years of leadership experience in not-for-profit management and services for older adults\, people with disabilities and family caregivers.  His work includes a focus on collaboration between health care providers and community service organizations to address social determinants of health\, quality of care and health outcomes. \nOrion previously served as President and CEO of CICOA Aging and In-Home Solutions\, the Area Agency on Aging in Indianapolis\, Indiana. This followed a career with the American Red Cross in a variety of leadership roles. Bell earned both MBA and BA degrees from the University of Louisville. \n \nDavid M. Bass\, PhD\, Senior Vice President and Senior Scientist  \nDr. David Bass is Senior Vice President\, Senior Research Scientist and Director of the Center for Research and Education at Benjamin Rose Institute on Aging. He has led over 80 funded research projects on topics such as: coping with chronic illness; family caregiving; quality of care and services; dementia and dementia care; and use and impact of formal services and informal support. Results of these investigations have been widely published in scientific and clinical journals\, such as the Journal of the American Geriatric Society and The Gerontologist. Results also have been used to develop innovative programs for older adults and their family members\, such as BRI Care Consultation™\, which has been delivered by over 60 licensed service organizations across the US. \n \nLisa Weitzman\, Director of Strategic Partnerships  \nLisa Weitzman is the Director of Strategic Partnerships at Benjamin Rose Institute on Aging. In this role\, she seeks to enhance Benjamin Rose’s role in the community through the development of local and national relationships. She also oversees the delivery of WeCare…Because You Do\, a family caregiver support program\, and has been instrumental in the launch of Nutrition Solution\, a program to address food insecurity and social isolation through medically-tailored meals for chronically-ill older adults. \nLisa earned a BA in history with a concentration in American Foreign Policy from Yale University\, an MA in Soviet Studies and International Economics from the Johns Hopkins School of Advanced International Studies\, and an MSW from Case Western Reserve University. She currently holds an LISW-S license and advanced certification in gerontology and case management. \n \nRachel Cannon\, MPH\, Senior Research Analyst  \nRachel Cannon\, MPH\, is a Senior Research Analyst at the Benjamin Rose Institute on Aging in the Center for Research and Education. She is the lead analyst in the initiatives that developed the current version of Best Practice Caregiving for professionals\, as well as the recently funded projects developing a new version for family and friend caregivers.
