DESCRIPTION:Join the Coalition to Transform Advanced Care and the American Heart Association for a webinar: \nImproving Quality of Life for People with Heart Failure through Integration of Palliative Care Services: A Pilot Project\nPalliative care can improve the patient’s journey in heart failure. Although heart failure is a serious illness\, people with heart failure have low rates of utilization.
